Basement ceiling vent won't stay up

I need help repairing my ceiling vent in my basement. This is a common enough problem.

The catch for me is that the entire vent sleeve that goes from the main conduit in the ceiling to the drywall is falling out. My vent differs from the videos I am seeing online because the tabs on the metal sleeve are outside the drywall. The screws go through the cover and the sleeve into the drywall, but of course, now the drywall has crumbled away. I had a professional try to repair it once, but they just used duct tape to secure the sleeve to the main vent. The sleeve and the cover are too heavy, and it eventually fell down again.

I can't figure out how to get the sleeve to stay up. It is too stiff to push the tabs to the other side of the drywall, and I don't think there is enough space between the drywall and main conduit anyway. I can't figure out how to use a piece of wood or anything else to screw into the other side of the drywall because I can't keep it in place when I push the sleeve into the ceiling.

I hope that makes sense. I am probably using the wrong terminology. Any suggestions?
